Any cat who exhibits signs of respiratory illness (runny eyes, nose, sneezing, coughing or wheezing) will not be admitted
without a vet's explanation of health issues (allergy, dental problems, etc.).  For obvious reasons we can not expose our
boarders to possible viral infections.

CONSENT FORM TO SEEK VETERINARY CARE:

As owner or guardian I give my full permission for Captain Kitt's or one of it's agents to seek medical care should my cat(s) require
attention/treatment/medication for any and all medical emergencies that may arise during their stay or any future stays at Captain Kitt's.  Captain Kitt's or
one of it's agents agrees to make all attempts to contact me and keep me abreast of any medical situation that may arise during their stay prior to and
after seeding medical care.

I give Captain kitt's and it's agents permission to transport my cat(s) to a board certified vet of their choice, should my vet be located outside a 10mile
radius of their boarding facility.  I further agree to reimburse chosen veterinary clinic for all expenses incurred for medical treatment(s) not to exceed
$500.00.  If treatment cost is expected to exceed this amount, I will fax or have my emergency contact sign a waiver if I choose to continue treatment.

Every attempt will be made to keep me informed of any issues as they unfold.  Clicking "accept" below as my choice in leiu of an ink signature confirms &  
binds the above statement as being true and agreed upon.
